We are a Registered Dietitian private practice. We provide Medical Nutrition Therapy (MNT), which is a focused and individualized nutrition service that utilizes therapeutic diet and education in conjunction with medical treatment to better help you manage your chronic illness and improve your health status.We also provide Diabetes Education for individuals with prediabetes, type 1&2 diabetes and women with gestational diabetes. Diabetes education is a critical element of care for all people with diabetes and for those at risk for developing the disease. It is necessary in helping individuals with prediabetes incorporate specific lifestyle changes needed to prevent the disease and for those who have diabetes to delay the complications associated with poor diabetes management. Effective diabetes self-management can be a significant contributor to long-term positive health outcomes. Differences in behaviors, health beliefs, culture, physical limitations as well as your emotional response to diabetes can have a significant impact on how you understand your illness and engage in self-management. We will assess your personal self-management needs and goals and together we will design a personalized education and support plan, based on your needs to help you maximize your health outcomes.

What to expect from your initial nutrition consultation

The initial consultation is a sixty to ninety minute long session where we will discuss your medical and diet history, referral diagnosis, medication and recent lab work. We will also discuss your food and physical activity records, and assess life-style and barriers to self-care. Finally, we will develop a plan of healthy eating and physical activity based on your nutritional needs, and implement strategies to aid in attaining and maintaining your health goals.

 

Insurance Information

Currently we accept AETNA, CIGNA, UNITED Health care, Medicare

Additionally, many private plans offer out-of- network reimbursement for Medical Nutrition Therapy and Diabetes Education. At the end of your visit, you will be provided with a “superbill”, which you may submit to your insurance company for out-of-network reimbursement. Please call your insurance provider to verify your benefits as reimbursement varies with each company’s policy.

 

What form of payments do we accept

In addition to reimbursement from private insurance providers we also accept cash, credit or personal check. Please note that payment is due at the time of visit.
